Important Notice: First things first, before you start to apply for the UMID Card, Let me know you that there’s no online registration application for obtaining the UMID Card. Even your family, friends or other proxies are not allowed to get your application.

It is required for you to have a personal appearance since you need to go through the process of biometrics data and interview for the verification of your personal details.



REQUIRED DOCUMENTS FOR UMID CARD MEMBER

1. SSS Members

    • Members have paid at least one (1) month contribution.
    • Valid date of coverage (Indicates when you started as an active SSS member).
    • No existing SSS ID /UMID Card.
    • For Kasambahay or House helpers without posted contribution/s:
        • Must be permanent the membership status of House Helper.

2. GSIS Members

    • All pensioners and GSIS active members are required to get eCard/UMID Card.

3. PhilHealth Members

    • UMID card is also available for those PhilHealth members and Indigent [see here the list of indigent] people who have insufficient income for family subsistence which is not covered by either SSS or GSIS members.

A. REGISTRATION FOR SSS MEMBERS

1. UMID Card Application Form

  • The SSS Number, Name (first name, middle name, last name), date of birth and gender/sex must be same on their database.
  • Fill out with your complete details including:
    • Use a black pen.
    • Provide your permanent address. (The address you provide will be the reference for delivery of the UMID card. So make sure to write the correct address)
    • Provide at least one (1) contact number (Mobile, Landline or e-mail address) to notify you of the status of your application.
